MyHeart MyLife is the Heart Foundation’s free support program for people living with heart disease and those who care for them. The program will help you understand how to best manage your condition for a healthier, happier life.

By joining, you’ll receive tailored information, expert guidance and practical tips via text messages, emails, videos and bite-sized articles. You’ll also be invited to join our active online community, where you can connect with others on a similar journey to you.

No matter where you are on your heart health journey, we’ll be with you every step of the way.

We're here for you

Being diagnosed with heart disease can feel lonely. By joining the MyHeart MyLife online community, you can connect with others who are on a similar journey and understand what you’re going through.

The Heart Foundation would like to acknowledge the many people, organisations and researchers who have contributed to the MyHeart MyLife program including the foundational work of Prof Clara Chow and her research team at Westmead Applied Research Centre from which evidence-based behaviour change strategies have been adapted for the MyHeart MyLife program.
